GP Referral

Research has shown the positive health benefits of regular exercise on our mind and body. However, despite this most of us need encouragement and support to take the first steps.

The fitness industry has been successful in enticing the healthy and active young adults to participate in exercise while other inactive groups have drawn limited attention. This is where the GP Referral Programme has the potential to play an important role in encouraging patients of all ages to increase their level of physical activity.

GP referrals can be for a variety of reasons, but they all have one thing in common - the person requires the services of a trained professional to design a safe and effective exercise programme.

The GP Referral Exercise Programme is a 12 week exercise programme for people who have any of the following medical conditions:-.

 
  • Angina
  • Obesity
  • Musculo-skeletal problems
  • Arthritis
  • Mild to moderate depression
  • Lower back problems
  • Asthma
  • plus many other conditions
 
A health professional e.g.GPs, Practice Nurses, Physiotherapists, Occupational Therapists etc. will complete a Pre Assessment Registration Questionnaire (PARQ) referral form which is based upon questions relating to your medical condition. On production of this questionnaire an activity programme will then be developed for you.

Referred individuals attend one of the the Easy Exercise Centres in either Winnington, Northwich or Lytham St Annes, Blackpool at supervised times to ensure safe and effective exercise.
